,week4  ,whereas a community health     
worker is a frontline public health     
worker who is a trusted member1 or has  
an unusually close understanding1 of the
 community served2 ,whereas this        
trusting relationship enables the worker
 to serve as a liaison1 link1 or        
intermediary between health and social  
services and the community1 facilitate  
access to services1 improve the quality 
and cultural competence of service      

delivery1 build individual and community
 capacity1 and increase health knowledge
 and self-sufficiency through a range of
 activities such as outreach1 community 
education1 informal counseling1 social  
support1 and advocacy2 ,whereas         
community health workers are a unique   
workforce1 recognized in #bg ,states    
with a professional certification2      
,whereas community health workers are a 
community-based workforce that builds   
relationships and capacity for health   
and well-being in underserved areas2    
,whereas community health workers are a 
historic and diverse workforce that goes
 back hundreds of years in the ,united  
,states and reflects the diversity of   
the ,nation2 ,whereas community health  
workers are known by more than #bej     
valued and respected titles1 including  
community health representatives1

outreach workers2 ,whereas community    
health workers are a cross-sector       
workforce that connects community       

members to health care and other social 
services1 reducing barriers to health   
and well-being2 ,whereas community      
health workers are a proven workforce   
with decades of research documenting    
effectiveness in maternal and child     
health1 chronic disease interventions1  
immunization1 oral health1 ,,hiv1       
primary care1 and many other            
disciplines1 and have a documented      
return on investment for many programs2 
,whereas sustainable funding of         
community health workers supports fair  
market wages and enhanced recruitment   
and retention of the workforce2 ,whereas
 community health workers fulfill a wide

providing cultural mediation among      
individuals1 communities1 and health and
 social service systems2  7#b7 offering 
culturally appropriate health education 
and information2  7#c7 offering care    
coordination1 case management1 and
