person-centered counseling to assist    
individuals in assessing their existing 
or anticipated long-term care needs and 
goals1 and developing and implementing a
 person-centered plan for long-term care
 that is consistent with the desires of 
such an individual and designed to meet 
the individual's specific needs1 goals1 
and circumstances2  7,c7 access for     
individuals to the full range of        
publicly-supported long-term care       
services and supports for which the     

individuals may be eligible1 including  
home and community-based service        
options1 by serving as a convenient     
point of entry for such programs and    
supports2 and  7,d7 in cooperation with 
area agencies on aging1 centers for     
independent living described in part ,c 
of title ,,vii of the ,rehabilitation

seq471 and other community-based

regarding available home and            
community-based services for individuals
 who are at risk for residing in1 or who

routines1 that can induce panic1        
anxiety1 and retraumatization as a      
result of experiences from the          
,holocaust4  7#d7 ,how supportive       
services providers may consider the     
unique needs of ,holocaust survivors4   
7#e7 ,how other services provided under 
that ,act1 as determined by the         
,assistant ,secretary for ,aging1 may   
serve ,holocaust survivors4  7c7 ,date  
of issuance ,the guidance described in  
subsection 7a7 shall be issued not later
 than #ahj days after the date of
