,,cases4 ,,by ,,richard ,h4 ,,johnston4 ,m4 ,d41 ,lecturer on ,laryngology in the ,university of ,maryland1 ,surgeon to the ,presbyterian ,hospital1 ,baltimore4 ,the esophagoscope has passed the experimental stage in the diagnosis and treatment of esophageal lesions4 ,its usefulness has been demonstrated so often that it would seem superfluous to dilate upon its value4 ,its use1 however1 is not as general as it should be4 ,there are still those who consider esophagoscopy unnecessary or impracticable4 ,at the ,presbyterian ,hospital we have had numerous instances of its practicability1 and with us it has become the routine practice to examine all patients complaining of obscure esophageal symptoms4 ,dr4 ,chevalier ,jackson records the case of a patient whose only symptom was a lump on swallowing4 ,she appeared to be a neurasthenic1 and his advice to have the esophagus examined was ignored by the family physician4 ,two months later1 with the patient etherized for a radical antrum operation1 he passed the esophagoscope and found a malignant growth4 ,three interesting cases have recently come under my observation1 and they illustrate so well the value of the esophagoscope ,i shall report them somewhat in detail4 ,the first patient was seen with ,dr4 ,e4 ,b4 ,freeman2 she was #fg years old4 ,the morning before she came to the hospital1 while eating ham1 she swallowed a large piece that had not been sufficiently masticated4 ,it lodged in the introitus esophagi and remained there4 ,when she came to the hospital she had swallowed neither solid nor liquid food for nearly thirty-six hours4 ,a half hour before examining the esophagus she was given a hypodermic of morphia and atropia4 ,with the patient in the sitting position the throat and upper end of the esophagus were anesthetized with #aj per cent solution of cocaine4 ,jackson's laryngeal speculum was introduced and the larynx pulled forward4 ,a large mass resembling somewhat an ulcerative epithelioma was seen1 and proved to be the piece of ham4 ,dr4 ,freeman and ,i removed it piecemeal with ,pfau's foreign body forceps4 ,it required about forty-five minutes to remove it entirely4 ,the patient stood the ordeal well1 and was able to go home the same afternoon4 ,for about a week she had temperature1 cough and expectoration1 but ultimately made a good recovery4 ,in this case the esophagoscope probably saved the patient an esophagotomy4 ,the second patient was a female1 thirty-three years old1 referred to me by ,dr4 ,j4 ,f4 ,chisolm1 of ,savannah4 ,while at an oyster supper she attempted to swallow a large oyster1 with the result that she choked for a few seconds and then had a sense of fulness in the region of the larynx4 ,the next day she had some difficulty in swallowing1 so that she took only liquids4 ,the second day afterward swallowing was decidedly painful2 she grew rapidly worse1 until the fourth day her condition was serious4 ,she reached this city the morning of the fifth day1 with a temperature of #ajj degrees and extreme prostration4 ,the examination of the esophagus was made under ether with the head in the extended position4 ,no foreign body was found1 but the upper end of the esophagus was red1 swollen and edematous1 and seemed to be closed4 ,the patient was given cold milk and ice bags to the throat4 ,for two days she suffered excruciating pain on swallowing1 and it looked as if we would have to resort to rectal feeding4 ,the next day there was slight amelioration of the pain1 which gradually disappeared4 ,in this case the esophagoscope enabled us to see at once that a foreign body was not present1 and that the symptoms were due to a severe1 acute inflammation1 probably caused by a piece of shell attached to the oyster4 ,i was asked by ,dr4 ,a4 ,m4 ,shipley to examine a patient who had been referred to him for probable cancer of the stomach4 ,the man was sixty years old and had had some trouble in swallowing for about two months4 ,attempts to pass the stomach tube were unsuccessful4 ,the patient was examined in the sitting position after cocaine anesthetization4 ,no difficulty was experienced in passing the esophagoscope4 ,about three inches below the cricoid cartilage the progress of the instrument was arrested by a tumor partially closing the esophageal lumen4 ,the esophagoscope showed that the tumor was too low for removal4 ,in this case the patient can be dilated through the esophagoscope and made more comfortable for the short time he has to live4 #iai ,north ,charles street4 ,,syphilitic ,,osteo-,,periostitis4 ,,by ,c4 ,g4 ,,moore1 '#ji4 ,senior ,medical ,student4 ,the bony manifestations of syphilis occur as secondary and tertiary lesions1 and as ,keyes1 of ,new ,york1 has pointed out1 these so-called 8nodes0 are simply local periosteal congestions1 accompanied by serious effusions without cell hyperplesia4 ,any bone in the body may be affected by syphilis1 but certain of them suffer by preference1 such as the thin bones of the nose and pharynx--that is1 those exposed to climatic changes and injuries1 such as the bones of the skull1 ulna1 tibia1 etc4 ,we must call special attention to injury as a powerful pre-disposing cause of bone syphilis1 for1 when we consider that bone lesions may be the only manifestations of existing syphilis1 with the presence of a bone lesion before us1 with an antecedent history of an injury1 we must not forget that we may overlook the true nature of the disease1 and hence must be constantly on the alert for the syphilitic taint4 ,lancereaux classified the bone lesions under three heads1 viz43 7a7 ,inflammatory osteo-periostitis4 7b7 ,gummy tumor of bone4 7c7 ,dry caries1 atrophic form4 7#a7 ,inflammatory osteo-periostitis is the most frequent form1 and is characterized by inflammatory phenomena1 vascularization and exudation of a serio-glutinous material4 ,it may be either diffuse or circumscribed1 and located1 as its name implies1 in the area of contact with the osseous and periosteal surfaces4 ,the pain is aching1 acute1 throbbing or boring in character1 while tenderness upon pressure and percussion is most exquisitely excruciating4 ,the diagnosis of inflammatory osteo-periostitis is comparatively easy1 if we remember the characteristics1 viz41 an oval1 painful1 boggy or even hard bony lesion1 accompanied by nocturnal exacerbations of pain1 with a concomitant or antecedent history of syphilis4 ,ostitis with parenchymatous thickening is somewhat less positive in its character1 but with nocturnal pains which are usually constant4 7#b7 ,gummy tumor of bone develops either under the periostum1 in the substance of bone1 or in the medullary canal4 ,it is simply an intensification of the process found in the inflammatory form just described1 the difference being that the cell hyperplasia is more abundant4 ,much of the new material collects in a circumscribed space1 and being more rapidly formed and less capable of organization1 it entails more profound lesions by its retrograde metamorphosis4 ,generally tumor of the bone is1 therefore1 a much more serious form of disease than osteo-periostitis4 ,in the long bones the medullary canal is the usual seat of deposit4 ,the bone becomes hypertrophied in a porous manner1 the ,haversian canals and canaliculi become enlarged and filled with a gummy material which resembles a solution of gum arabic4 ,in the flat bones1 especially the cranial bones1 the cancellar tissue is attacked1 and may cause a separation of the two tables1 and often necrosis of one or the other plates results4 ,if it happens to be the inner one which undergoes carious degeneration1 brain symptoms will develop4 7#c7 ,dry ,caries4--,according to ,virchow1 dry caries is always due to syphilis4 ,this affection is a miniature gummy ostitis4 ,around one of the vascular canals the gummy material is deposited1 this gummy material being later absorbed1 leaving a stellate induration4 ,this goes on leaving a funnel-shaped depression1 its point leading into the diploe1 which may be plainly appreciated by palpation4 ,the essential features of this caries is the fact that no sequestra are formed1 no pus extruded1 nor is the skin but rarely involved4 ,the following is a case of syphilis in which osteo-periostitis developed3 ,on ,january #bbnd1 #aiji1 ,r4 ,b41 age #ce1 white1 a housewife1 applied to the ,medical ,department of the ,university ,hospital ,dispensary for treatment1 complaining of rheumatism in her back and a sore shin4 ,the patient has been married #ad years and has had four children2 the two eldest are the only ones now living4 ,three years ago she gave birth to a full-term child which only lived a few minutes after expulsion4 ,one year later she gave birth to another child1 which was not at full term1 but about six months advanced4 ,she says two days previous to this birth she fell down stairs and struck on her abdomen4 ,when the child was born its thigh was fractured1 and the physician who attended her said the fracture was caused by the fall4 ,her father died nine years ago1 at the age of #fh years1 of apoplexy2 her mother was killed a few years ago in an accident4 ,she has two brothers and three sisters1 all living and in good health1 as far as she knows4 ,she is at present living with her husband1 and says he is apparently well and sound1 but drinks heavily1 and when under the influence of liquor abuses her a great deal4 ,patient denies ever having had tuberculosis1 syphilis1 diphtheria1 typhoid1 scarlet fever1 malaria1 grippe1 gonorrhoea1 or any of the nervous or malignant diseases4 ,she sometimes has a sore throat when she takes cold1 but it only lasts a few days4 ,she has complained of rheumatism in her back and limbs for the past three or four years1 and thinks it is worse at night4 ,patient never complained of any trouble other than those mentioned until three years ago1 when her third child was born4 ,she says that at that time her hair fell out1 and an eruption1 which itched slightly1 broke out all over her body1 including her face1 but only extended down her arms as far as her wrists4 ,this lasted a few weeks1 then seemed to subside1 but never entirely disappeared1 and when she gave birth to the still-born child1 one year later1 it broke out again worse than before4 ,she went to ,dr4 ,mc,elfresh1 who treated her for about three weeks1 giving her some medicine to take internally1 also some sulphur ointment4 ,she for a time got some better1 but owing to her circumstances was unable to continue treatment with ,dr4 ,mc,elfresh1 and has done nothing for her condition until the present time4 ,for the past four months she has been suffering with a pain in her right shin2 this has been gradually getting worse1 and one week ago began to swell and cause her considerable pain1 being worse at night1 and sometimes hurting her so much that she is unable to sleep1 hence her reason for coming to the dispensary4 ,upon questioning her in regard to her general health1 she says she feels as well as she ever did1 with the exception of the previously mentioned pain4 ,her appetite and digestion are good1 and her bowels are regular1 and she has no lung1 heart or kidney trouble4 ,she has had no headaches1 nausea1 or vomiting1 and her menstrual periods have always been regular and painless4 ,upon examining patient ,i found her to be well nourished and well developed1 weighing #adb pounds4 ,her color was good and her pupils about normal in size1 reacting to light and accommodation4 ,both patella reflexes were absent1 also ,romberg's sign1 and there was no enlargement of the mastoid1 epitrochlear1 post-cervical or inguinal glands4 ,her pulse was #hd to the minute1 regular in rate and force4 ,the tension and volume was good2 her temperature was #ih4f4 ,her heart was normal in size1 and on auscultation the sounds were clear and no murmurs were heard4 ,the expansion of both lungs was good1 the respirations being #bj to the minute4 ,percussion and auscultation were negative2 all the abdominal organs seemed to be normal4 ,there was an ecchymotic area under her left eye1 which she says was caused by her husband striking her two years ago when intoxicated4 ,there was a circular reddish macular papular1 non-itching eruption which does not disappear on pressure1 varying from the size of a bird shot to that of a nickel1 and is confined to her back1 chest1 shoulders and arms1 most profuse on the left side1 and is not seen on the lower limbs at all4 ,on her left shoulder some of these lesions have developed into pustules1 which have become infected and slightly ulcerated2 these give her slight pain4 ,in the corner of her mouth is a scar which looks like the initial sign of lues1 but she claims it appeared a few weeks after the breaking out on her body4 ,on examining her mouth no mucus patches or scars were found4 ,she now has an osteo-periostitis on the anterior aspect of her right tibia4 ,it is moderately swollen1 slightly reddened1 and is very painful 7the pain is aching1 acute and boring in character7 on pressure1 and on tapping the bone with my finger above and below this point it caused her intense pain4 7,patient claims she has never received any injury in this location47 ,upon consulting ,dr4 ,mc,elfresh1 he remembered the case and said that he had treated her for a short time about two years ago for the initial symptoms of syphilis1 but since then has never seen her4 ,she is now receiving the mixed treatment of protiodid of mercury1 gr4 1 with a saturated solution of potassium iodid three times a day1 starting her on ten drops1 then increasing it one drop each time taken4 ,i requested her to return when the medicine is finished4 ,,discussion ,,by ,,dr4 ,,warner ,,holt1 ,,of ,,washington1 ,,of ,,the ,,paper ,,on ,,the ,,chemical ,,co-,,relation ,,between ,,the ,,salivary ,,glands ,,and ,research1 on ,december #af1 #aijh4 ,dr4 ,holt said in part3 8,this experimental study by ,dr4 ,hemmeter is not1 as it might appear to be1 only an inquiry into the physiology of a limited part of the digestive apparatus1 but it is an attempt to solve a biologic problem and to get at the broad basic principles that underlie the chemical co-relation of the organs4 8,when a worker occupies himself with the effect of the extirpation of one organ of digestion upon the organs in the next segment of the digestive apparatus1 he naturally thinks of phenomena of exclusion or loss of function in one or the other of the segments following the one extirpated1 but instead of phenomena of exclusion it is conceivable that those of exaggerated activity in the other segments of the digestive tube might result4 8,for 'a priori' we cannot know whether the influence that one segment of the digestive tract exerts upon the succeeding segment is that of stimulation or of inhibition1 or of both1 viz41 of stimulation under one set of conditions and inhibition under another set of conditions4 ,in the investigation of the problem of a chemical co-relation between the salivary glands and the stomach1 ,dr4 ,hemmeter has done meritorious work1 a great part of which it has been my good fortune to observe and assist in personally2 though ,i am a physician in the employment of the government at ,washington1 ,i consider myself a post-graduate student of ,professor ,hemmeter4 ,i have seen personally four of his animals that had successfully been nursed through the ,pawlow operation and extirpation of the salivary glands after months of the most trying work4 ,it required a great deal of perseverance to persist in this kind of work1 especially when some of the best animals that had emerged safely from the vicissitudes of the operation for an accessory stomach and from the removal of all the salivary glands on one side of the head succumbed to the third operation in the attempt to remove the remaining salivary glands on the other side4 8,the history of these operative failures1 though they will never be told1 constitute a large part of the merit of those who have worked with ,dr4 ,hemmeter in this research4 ,no matter what the final outcome of the future investigation of this problem will be1 whether affirmative or negative1 the intrinsic value of such work will be appreciated by all who are to the least degree conversant with the history of physiology4 ,nowadays we are too liable to forget the hard plodders in experimental work who have started the solution of a problem1 and when the last word has been said the worker of the beginning is generally forgotten4 8,in this connection ,i desire to quote an expression of ,prof4 ,william ,h4 ,welch concerning the merit of the work of ex-,surgeon ,general ,sternberg1 done since the first ,yellow ,fever ,commission was appointed1 in #ahgi 7see ,medical ,news4 ,june #ba1 #aijb4 p4 #aaih74 ,dr4 ,welch said 'that ,sternberg's work with yellow fever would stand forever2 that it was a common thing in these busy days to forget the steps which led up to an important discovery4 ,all that ,dr4 ,sternberg had done in the study of yellow fever was necessary work1 and it had to be done just in the way that he did it4 ,the ground had first to be cleared4 ,if it were not so1 the discovery had not been possible2 and later discoverers themselves would have had to hunt out the large host of microorganisms which ,dr4 ,sternberg had described and laid aside4' 8,and similarly ,i can say of ,dr4 ,hemmeter's efforts that1 no matter what the eventual outcome of this problem will be1 all that he has done was necessary work1 and it had to be done just in the way that he did it4 8,just one more idea and ,i shall have finished4 ,it concerns the demonstration of such research work in places at a distance from the experimentor's laboratory4 ,such demonstrations are always attended with great difficulty4 ,they usually require four animals1 two or three janitors to transport them1 and as many laboratory assistants as the director of the laboratory can manage to take with him4 ,the technique of these operations1 the high-grade sensitiveness of operated animals1 the refinement with which chemical tests should be made1 all require for their safe conduct that the experimentor should work only with those men who are used to his system4 ,the animals themselves are always influenced in one way or other by the presence of strangers4 ,i remember in one animal which was demonstrated on ,march #agth1 at the ,university ,hospital1 the demonstration at which ,dr4 ,satterthwaite was present1 a most unexpected change in the quality of the gastric secretions took place4 ,this was a control animal which had undergone no operation whatever4 ,he was simply taken along to show the proteolytic power of a normal dog and compare it with the operated dogs4 ,his gastric juice had been previously tested on several occasions1 and always found to be of regular standard1 but on the night of the demonstration before the ,medical ,society this animal's gastric juice was practically inactive1 containing no ,,hcl nor pepsin4 8,dr4 ,hemmeter has already informed you that in some animals the loss of gastric juice after extirpation of the salivary glands is only temporary1 and that in varying time--in some cases three weeks1 in other animals three to four months--there is a gradual resumption of gastric secretion4 ,this resumed secretion1 however1 never becomes as effective as it was in the same dog before an operation4 ,the question when to begin to make observations on an operated animal depends entirely upon the state of this animal2 if the dog eats his food with appetite1 he has no fever1 and his digestion appears to be satisfactory2 then the observations may be begun1 even if it is only one week or ten days after the last operation4 ,one of the most valuable animals that was used in this series of experiments was so injured in the effort to transport him to another laboratory that he could not be used for further experimentation4 ,the dog struggled so in his holder while he was being transported in a wagon that the partition of true mucosa which separates the accessory from the plain stomach was broken through4 ,this had happened once before in transporting a dog from the laboratory to ,dr4 ,hemmeter's country place1 and his associates in the ,medical ,faculty1 becoming aware of the great labor and cost involved in such operations1 and the rarity with which they succeed1 advised that no further ,pawlow dogs be sent to other laboratories40 ,,an ,,interesting ,,case ,,of ,,scrotal ,,hernia4 ,,by ,g4 ,e4 ,,bennett1 '#ji4 ,senior ,medical ,student4 complaining of great pain and discomfort in the right inguinal region and in the scrotum of the same side4 ,physical ,examination--,inspection showed an enormously enlarged scrotum1 more marked on the right side1 and a prominent swelling along the right inguinal canal1 which was most marked when standing4 ,marked discoloration on the skin of the scrotum and inner side of both thighs1 probably due to use of counter-irritants4 ,palpation--,mass soft and freely movable1 showing no skin attachments2 slight impulse on coughing4 ,slightly painful on pressure4 ,some gurgling when manipulated4 ,percussion--,slightly tympanitic4 ,remarks--,contents of the sacs were forced into abdominal cavity after prolonged manipulation1 returning to original condition as soon as pressure was taken away4 ,history of patient shows nothing of interest except that of the present condition1 which began suddenly twenty years ago4 ,while lifting a heavy block of stone had a feeling as though something had 8given away0 in his right side4 ,this sensation was immediately followed by one of intense pain and general discomfort4 ,the day following the patient noticed a small lump in the right inguinal region that disappeared on pressure1 returning when he lifted any heavy object4 ,for eighteen years the condition gave him no serious discomfort except for the wearing of a truss and becoming larger4 ,two years ago the truss was discarded as being useless4 ,one week ago conditions grew suddenly worse1 and patient was confined to bed4 ,has suffered a great deal of pain and has been unable to sleep4 ,on ,january #bb1 #aiji1 patient was operated upon by ,professor ,winslow4 ,operation as follows3 ,patient was brought to the operating room at #aa4cj ,a4 ,m41 anesthetized and prepared for an aseptic operation4 ,incision about five inches in length was made parallel to ,poupart's ligament and immediately over the inguinal canal1 passing through the skin and subcutaneous fat4 ,the external ring having been exposed a grooved director was passed into same1 passing under the aponeurosis of the external oblique muscle2 fibers of same were split1 using the director as a protective guide4 ,the sac was exposed and carefully dissected free from its surrounding tissues1 and upon examination was found to be continuous with the covering of the testacle 7giving the appearance of a congenital hernia74 ,the sac was next opened and found to contain small intestines and a ,meckel's diverticulum4 ,following this the intestines were replaced in the abdominal cavity4 ,digital examination through the internal ring showed the bladder to be adherent to the peritoneum at the margin and toward the median line4 ,the sac was tied close to the internal ring1 cut free4 ,the distal portions of the sac were drawn upward1 bringing the testacle into view2 sac was cut close to same and sutured so as to enclose the greater part of it4 ,the margins of the internal oblique and transversalis muscles were sutured to ,poupart's ligament by a mattress suture4 ,the aponeurosis of the external oblique was re-established into normal position by suturing1 and the skin closed by subcutaneous silver wire suture silk having been used for all other sutures4 ,sterile dressings were then applied and cardboard splints to keep limb immobile4 ,then bandaged with crenolin4 ,the patient left the operating room at #a ,p4 ,m4 in good condition4 ,notes of ,interest-- ,that a hernia1 apparently congenital1 should not have made an earlier appearance4 ,the presence of a ,meckel's diverticulum in the sac1 this being the condition that gives rise to a true ,richter's hernia4 ,that a hernia of so large proportion should have caused so little trouble to the patient4 ,patient recovered in very short time1 leaving the hospital in good condition4 ,,the ,,teaching ,,of ,,the ,,specialties4 ,,by ,,hiram ,,woods1 ,m4 ,d4 ,read ,before the ,university of ,maryland ,medical ,association1 ,april #ba1 #aiji4 ,teaching specialties to undergraduates must be carefully separated from the same teaching to post-graduate students4 ,the latter may be supposed to have given such thought to their future career as to have reached the conclusion that they wish to devote themselves to a certain line of work4 ,it is not these men whom ,i propose to discuss1 save to say1 in passing1 that the average six weeks' or two months' course offered by post-graduate schools is totally inadequate4 ,as a rule such courses attract a large number of men who do little more than follow the clinical work of some well-known specialist and pick up what they can4 ,either prolonged hospital experience1 as interne or clinical assistant1 or a special individual course1 with a competent teacher1 is needed for a decent foundation in special work4 ,it seems to me that with undergraduate work the first important step is to secure the attention of the men1 and convince them that there is something in the special course worthy of notice4 ,there is a state of mind1 not unnatural to the undergraduate1 regarding specialties4 ,most of them propose to begin professional life as general practitioners4 ,they think they will never have use for knowledge in the so-called specialties1 and that the little they will need in order to get a passing mark can be easily crammed at the close of the session4 ,as a matter of fact1 it is easy to frame questions so that this 8crammed0 information is of little use1 and the most liberal grading hardly ever brings such men up to a passing mark4 ,the mental attitude of these men is unjust to themselves1 their teacher and the patients who1 in the near future1 will entrust to them their physical welfare4 ,the very term 8general practitioner0 implies a general knowledge of medicine4 ,if one lives in a city1 where the services of specialists are readily obtained1 he may1 if he desires1 refuse certain cases1 and take only such as he wants--say such as belong to internal medicine4 ,but in so doing he becomes a specialist himself1 and if he has neglected a properly prepared special course for undergraduates while a student he will miss information of great use to him as an internist4 ,many men1 however1 do not practice where specialists are easily obtained1 and1 perforce1 must take cases which would logically come under one or other of the recognized specialties4 ,here is a professional responsibility which it is the aim and duty of a special teacher to enable his student to meet4 ,may ,i illustrate by directing attention to two troubles which it is my own privilege to explain to our students8 ,iritis1 in eye1 and acute otitis media1 in ear diseases1 are very common troubles4 ,both are1 as a rule1 readily diagnosed1 and both offer good prognosis4 ,yet the responsibility for eyesight in one1 and may be life in the other1 may depend on the diagnostic ability and therapeutic resources of the man who first sees the case4 ,men are blind and children dead because a general practitioner has not known enough of what was offered him when a student to make a correct diagnosis4 ,the red eye has been called conjunctivitis and treated with nitrate of silver1 in spite of the absence of purulency1 while the small1 inactive pupil has escaped notice4 ,the ear pain has been called 8earache0 without an aural examination1 a hops bag and opium have been ordered1 and the doctor has gone home with that false sense of security which is so dangerous4 ,why8 ,if his teacher has known his business1 it is not because he was not told how to look for iritis and acute inflammation of the drum1 but because he had not given the subjects sufficient thought to get them drilled into his thinking apparatus4 ,probably he could tell the diagnostic points of iritis1 if asked1 or the signs of ear-drum inflammation2 but this was 8crammed0 information1 not part of his real knowledge4 ,if it is the duty of the student1 anticipating general practice1 to think about the specialties taught during his undergraduate course1 it is still more the duty of his teacher to present him only such things as the general man needs4 ,he will make a grievous blunder if he tries to make specialists of his men4 ,his selection of subjects should be limited to the diseases which are of common occurrence1 and stress should be laid on diagnosis4 ,if one knows1 first1 what to look for1 and secondly how to recognize signs and symptoms1 he will generally find proper treatment4 ,troubles which would lead the patient to go to the specialist primarily1 without consulting his family physician1 should receive little attention4 ,two other classes of lesions in such organs as are usually handed over to specialists should receive attention in the undergraduate course--those which are apt to cause remote or reflex disturbances1 and those which are definitely symptomatic of central lesions4 ,the first should1 in my judgment1 be dwelt upon only to such an extent as to enable the student to know causative relation and method of diagnosis4 ,treatment1 unless very simple and easily within the reach of the medical man--i4 e41 the general practitioner--should be given little time4 ,as to the second class1 every neurologist knows that ,tabes ,dorsalis would often be diagnosed early1 and proper treatment instituted1 if the physician had known the meaning of association of gastric crises with ,argyll-,robertson pupil1 and had seen enough of these things to have them in his every-day thoughts4 ,one could present many other illustrations1 but this shows what is meant4 ,with the student convinced of the necessity of thinking about the specialty taught1 the instructor careful in selection of his subjects1 a duty rests on those who fix the curriculum4 ,it is unfair to students to use four years for work which can be done in three4 ,i believe that the object of the establishment of a four-year course was to furnish a year in which students1 freed from the responsibility of examinations1 might have time for guidance in clinical observation4 ,the ideal plan1 in my judgment1 is to get rid of didactic lectures and examinations by the end of the third year1 and to devote the fourth to clinical observation4 ,genito-urinary work1 gynaecology1 rhinology1 neurology1 ophthalmology1 otology1 cannot be learned from text-books or lectures1 at least in such a way as to become integral parts of a man's daily thinking4 ,and to my mind this is the only special information which will help the general practitioner in his daily work4 ,a few hours spent in a large clinic brings more instruction than a whole year of lectures4 ,the personal contact of man to man1 the exchange of thoughts and impressions1 are what sink in4 ,a student is not to be blamed if he fails to attend these opportunities when he knows that in a few weeks he must face the ordeal of examinations on the didactic work1 and that the result of these will determine his graduation4 ,to put into a few words my idea of teaching specialties--it is the duty of the student to realize that nothing is put into the undergraduate course which is not important to him2 it is the duty of the teacher to select only what is important to the general practitioner2 it is the duty of the school authorities to so arrange the curriculum as to give students enough time to observe special practice personally1 in small sections1 so that what is taught may be so impressed by observation as to become a real factor in their medical thought4 ,a ,,case ,,of ,,sporadic ,,cretinism4 ,,by ,e4 never evinced any disposition to play or converse with the other children4 ,i saw this child on the #dth day of ,january1 #aijh1 in a purely accidental manner4 ,the parents had been told by their attendant that the child had either rickets or was an idiot1 and they in consequence had kept the child in the background for two or three years1 being very much chagrined and mortified at the prospect of bearing through life the burden of hopeless idiocy4 ,the child had such classical symptoms of cretinism that ,i asked permission of the family to treat him for awhile1 though it required some persuasion1 because of the fact that they felt it was time and money wasted4 ,on the #eth day of ,january1 #aijh1 the child was five years and a half old1 twenty-eight inches in height1 circumference of chest twenty-one1 abdomen twenty-three4 ,he was given one and one-half grains of thyroid extract twice daily1 the dose being gradually increased until he showed signs of irritability1 with accelerated pulse4 ,the child's extremities soon warmed up1 the circulation became better1 the hair began to grow1 the child for the first time in its life walked and talked1 began to take note of surroundings and to play with the other children4 ,just one year after the beginning of the treatment--,january #e1 #aiji--the child was thirty-five and three-quarter inches in height1 chest in ,england1 on the ,continent and in ,america1 showing that the disease is not confined to any one country4 ,while the disease is comparatively rare1 cretins are more common than was formerly supposed4 ,the disease seems to be in reality a pachydermatous cachexia1 and it is now1 ,i believe1 well established that it is caused by congenital absence of the thyroid gland or to the presence of something which abolishes its functions4 ,little is known as to the causes of its destruction or abolishment of function4 ,as a rule only one case occurs in a family1 the other members presenting nothing abnormal in their mental or physical development1 hence the term sporadic4 ,it has been more frequently reported in the ,tyrol1 in ,switzerland1 a coincidence which makes this child's case all practically identical with those of the myxoedema which follows the removal of the thyroid gland in adults4 ,the symptoms of cretinism in most cases in infants make their appearance during the first year1 occasionally1 however1 not until the child is three or four years of age4 ,the appearance of the cretin is very striking1 and so characteristic that when once seen the disease can hardly fail to be recognized4 ,the child is much dwarfed1 the fingers and toes are short and stumpy1 the cutaneous tissues seem to be thick and boggy1 but do not pit on pressure1 as in ordinary oedema4 ,the facies is extremely characteristic4 ,the head seems large for the body1 the fontanel is open until the eighth or tenth year1 the forehead is low and the base of the nose broad1 so that the eyes seem unusually wide apart4 ,the lips are thick1 the mouth half open and the tongue protrudes slightly1 the cheeks are baggy and the hair is coarse1 short and straight1 and the skin has the peculiar leathery feel of elephant skin4 ,the abdomen is pendulous1 large1 streaked with prominent veins1 and reminds one of rickets4 ,the skin is dry1 the voice husky and rough4 ,there is but one word which describes the peculiar clumsy manner of walking--that word is waddle4 ,the child actually waddles like a duck4 ,the temperature is always subnormal1 and one of the things the mother will always call to your attention is the fact that the child has such cold hands and feet and requires so much more cover than the other children4 ,cretins are dull1 placid and good natured1 never quarrelsome4 ,treatment--,there is no tendency toward spontaneous improvement4 ,these cases have until the last few years been considered hopeless and condemned to a life of idiocy4 ,really1 in the treatment of cretinism in the adult marvelous results have been got from the administration of the dried and dessicated extract of the thyroid gland of the sheep4 ,this has led to its use in the myxoedema of infancy4 ,the results are astounding4 ,the child grows mentally and physically1 takes note of surroundings to which it formerly paid no attention whatever1 and can be taught almost as well as a perfectly normal child4 ,in all cases the thyroid extract must be kept up indefinitely1 the dose being gradually increased1 otherwise the improvement ceases at once4 ,,items4 ,at the commencement of ,pathologically ,considered10 at the coming meeting of the ,american ,medical ,association4 99 99 99 99 99 ,the ,council on ,medical ,education of the ,american ,medical ,association in its annual report has this to say concerning college mergers3 ,another encouraging fact to be noted is the mergers being made among medical schools whereby stronger schools are resulting4 ,notably in ,indiana1 all of the regular schools in the state merged into the medical department of ,indiana ,university1 while in ,kentucky all of the medical schools merged into the ,university of ,louisville4 ,in ,cincinnati the two regular schools merged into the ,university of ,cincinnati2 in ,minnesota ,hamline merged into the medical department of the ,university of ,minnesota4 ,there are numerous other cities where mergers might be brought about if those interested in general education and those in medical education in each city would work together to secure them4 ,for example1 if all the medical colleges of any large city1 such as ,chicago1 ,philadelphia1 ,st4 ,louis or others1 could be merged into one great university medical school1 such as are to be found in ,berlin1 ,paris or ,vienna1 it would be of the greatest possible advantage to medical education in ,america4 ,in the evolution of general and medical education in this country it is becoming more and more evident that a well-rounded university needs a strong medical department1 and it is now equally clear that a medical school cannot reach the highest stage of its development except as the medical department of a strong university4 ,it is evident that within a few years the medical schools of this country will1 with few exceptions1 be the medical departments of universities4 ,fortunately for the medical school1 the university needs the medical school quite as much as the medical school needs the university1 so that almost any independent medical school of real merit can secure desirable union with a university4 ,and this change will solve most of our present problems in medical education4 ,since our last conference there have been five important mergers of medical colleges by which nine medical schools are replaced by four stronger ones4 ,these mergers were as follows3 #a4 ,at ,louisville1 ,ky41 the ,louisville and ,hospital ,medical ,college1 the ,kentucky ,school of ,medicine and the ,university of ,louisville ,medical ,department united1 retaining the name of the ,university of ,louisville ,medical ,department4 ,this leaves but one regular medical college in ,louisville1 where there were five colleges two years ago4 ,as a direct result of this merger1 the school has received #be1jjj from the city of ,louisville1 and steps have been taken to build a new city hospital1 which is to be largely under the control of the medical school4 #b4 ,at ,cincinnati the merger between the ,medical ,college of ,ohio and the ,miami ,medical ,college has been completed1 the new school to be the ,medical ,department of the ,university of ,cincinnati4 ,the building of an enormous new city hospital has already been started near the university campus1 and a new medical college building will be erected adjoining this hospital4 ,the outlook for this new school is very encouraging4 #c4 ,the ,keokuk ,medical ,college1 ,college of ,physicians and ,surgeons1 located at ,keokuk1 ,iowa1 has turned all its property and good will over to the ,drake ,university1 ,college of ,medicine1 at ,des ,moines1 ,iowa4 ,amalgamation of the ,cooper ,medical ,college with ,leland ,stanford ,university is announced4 ,henceforth the ,san ,francisco institution will be designated the ,school of ,medicine of ,stanford ,university4 ,the affiliation was given approval sometime ago1 and it only remained for the board of trustees of the ,university to formally accept the gift4 ,why can't the independent medical colleges of ,baltimore come together8 ,such an event would accrue to the best interests of all concerned1 and would greatly tend to eliminate ,baltimore as one of the dark spots upon the medical educational horizon4 ,,nurses ,,win ,,diplomas4 ,in spotless white and amid a bower of flowers1 #af pretty young women were handed their diplomas yesterday as graduates of the ,university ,hospital ,school for ,nurses by the ,dean1 ,prof4 ,r4 ,dorsey ,coale4 ,there were #ag nurses to graduate this year1 but one of them1 ,miss ,catherine ,m4 ,dukes1 is seriously ill and could not attend4 ,after the conferring of degrees ,dr4 ,arthur ,m4 ,shipley gave the young nurses advice as to their future4 ,the opening prayer was delivered by ,rev4 ,edwin ,b4 ,niver1 rector of ,christ ,protestant ,episcopal ,church1 and benediction was pronounced by ,rev4 ,dr4 ,hemsley1 of ,oakland1 ,md4 ,the hall of the ,university was crowded with friends and relatives of the graduates4 ,it was decorated with carnations and potted palms1 and around the pillars was twined black and red bunting1 the ,university colors4 ,the nurses1 preceded by ,professor ,coale and ,dr4 ,shipley1 entered the hall in pairs1 carrying bouquets of ,marguerites4 ,dr4 ,shipley said that much of the nurses' training had been under his supervision1 and he felt a personal interest in them4 ,women1 he said1 invariably scared him1 but someone informed the physician that was not always so1 for ,dr4 ,shipley is to become a benedict today4 8,you have chosen a work that is second to none in the world10 said ,dr4 ,shipley4 8,you have before you possibilities that are almost limitless4 ,you are on the threshold of a life that is to be of your own making1 for the chief danger of the individual nurse is drifting4 ,it is so easy to forget the old-time standards and call them old-fashioned4 ,old-fashioned they may be1 but they have stood the test of generations of correct living and thinking40 ,at night the graduates were given a farewell reception and dance by the undergraduates4 ,dispensary ,report1 ,april1 #aijh1 to
